1. Department of Social Welfare (JKM) Assistance

The Jabatan Kebajikan Masyarakat (JKM) — Department of Social Welfare — provides several forms of assistance for elderly Malaysians:

  • Bantuan Orang Tua (BOT) — A monthly cash assistance of RM 500 for elderly Malaysians aged 60 and above who have no source of income and no family support. Applications are made through the nearest JKM district office.
  • Government-run care homes — JKM operates Rumah Seri Kenangan (elderly care homes) that provide free accommodation and care for eligible elderly citizens. Eligibility requires referral from JKM and is based on financial need.
  • Community-based care — JKM supports community programmes such as Pusat Aktiviti Warga Emas (PAWE), which provide daytime activities, meals, and basic health services for elderly individuals in the community.

2. EPF Withdrawal for Medical Purposes

The Employees Provident Fund (EPF / KWSP) allows members to withdraw funds from their Account 2 for medical purposes, including the treatment of critical illnesses for themselves, their spouse, or their parents. While this does not cover nursing home fees directly, it can help with significant medical expenses associated with elderly care.

To apply, members need to submit the relevant medical documentation to EPF. Processing typically takes 5 to 14 working days.

3. Tax Relief for Medical Expenses

The Malaysian government provides tax relief for individuals who bear medical expenses for their parents:

  • Medical expenses for parents — Up to RM 8,000 tax relief for medical treatment, special needs, and carer expenses for parents. This includes nursing home fees if they involve medical or nursing care.
  • Full medical examination — Up to RM 1,000 tax relief for complete medical examinations for parents (included within the RM 8,000 limit).
  • Parental care tax relief — RM 1,500 per parent for taxpayers who are caring for parents aged 60 and above with a monthly income not exceeding RM 24,000 per year.

These deductions can help offset the cost of elderly care when filing annual income tax returns.

4. SOCSO and PERKESO Benefits

For elderly individuals who were previously employed and contributed to SOCSO (PERKESO), certain benefits may be available including invalidity benefits, medical assistance for work-related injuries, and rehabilitation support.

5. Zakat Assistance

For Muslim families, Zakat institutions in each state provide financial assistance to eligible elderly individuals under the asnaf (eligible recipient) categories. This can include monthly financial aid, medical assistance, and welfare support. Applications are made through the respective state religious authorities (Majlis Agama Islam Negeri).

6. Private Insurance and Medical Cards

If your elderly parent has a medical insurance policy or medical card, check the coverage terms. Some policies cover:

  • Hospitalisation and surgical expenses
  • Rehabilitation services including physiotherapy
  • Home nursing care (some policies)
  • Outpatient treatment and specialist consultations

Review the policy carefully and speak to the insurance provider about what is claimable for nursing home or elderly care services.

7. Community and NGO Support

Several non-governmental organisations (NGOs) in Malaysia provide support for elderly care:

  • Alzheimer's Disease Foundation Malaysia (ADFM) — Support, education, and resources for families dealing with dementia.
  • Malaysian Healthy Ageing Society (MHAS) — Programmes and advocacy for healthy ageing.
  • National Council of Senior Citizens Organisations Malaysia (NACSCOM) — Advocacy and welfare programmes for senior citizens.
